Common Qualifying Conditions:
Anxiety, Depression, PTSD, ADHD/ADD, Insomnia, Social Phobia, Panic Disorder, Bipolar Disorder, OCD, ASD, Nervousness, and others.
To Qualify:
• You must have a qualifying mental health condition that is significantly helped by the presence of your animal companion
• You must be 18 years or older
Important Information:

Missed Appointments:
If you miss your appointment or do not answer at your scheduled appointment time, you may not be eligible for a refund.
Multiple Animals:
One animal per appointment. Under housing law, each emotional support animal must provide its own distinct therapeutic benefit or assist with a different qualifying condition. A separate appointment is required for each animal.
