After purchase, you will receive an email to schedule your virtual appointment with Dr. Polzin.
What’s Included:
• ESA letter provided same day as your appointment (if appropriate)
• Signed by licensed physician Dr. DJ Polzin
• Valid for housing for up to 1 year after issuance
• Will exempt patient from pet deposits, and “no-pet” housing restrictions (including dorms)
• Affordable, legitimate physician-led evaluation
Common Qualifying Conditions:
Anxiety, Depression, PTSD, ADHD/ADD, Insomnia, Social Phobia, Panic Disorder, Bipolar Disorder, OCD, ASD, Nervousness, and others.
To Qualify:
• You must have a qualifying mental health condition that is significantly helped by the presence of your animal companion
• If you do not have a prior diagnosis please make sure to look up DSM criteria for the diagnosis you believe you have. For example Google: "DSM diagnosis criteria for Anxiety". If you fit those criteria and your symptoms are considered debilitating you will be approved.
• You must be 18 years or older
Important Information:
• Psychiatric Service Animals and Emotional Support Animals are different. Please see my related blog article to learn more.
• ESA letters are valid in all U.S. states except California
Missed Appointments:
If you miss your appointment or do not answer at your scheduled appointment time, you may not be eligible for a refund.
Multiple Animals:
One animal per appointment. Under housing law, each emotional support animal must provide its own distinct therapeutic benefit or assist with a different qualifying condition. A separate appointment is required for each animal.
